Please provide proof of your legal name change in the form of an updated state ID, updated passport, court document or state issued marriage certificate and email to hrsc@jhmi.edu
Along with the required documentation, please include how you wish for your name to display:
- First:
- Middle (Initial or full name):
- Last Name:
Once your name has been updated and you see it reflected in ESS:
- If participating in the tuition benefit, the name should also be updated at the university so that the name in the student portal/on supporting documents matches the name in the employee record.
- To obtain an updated insurance card, you would contact the insurance company at the number on the back of your cards once your updated name reflects on your pay statement.

- A user can update their name in EPIC through the epic help desk. If you go to my.jh.edu and type in Epic it will give you two resources
